What To Expect At Your Visit

1. Book your appointment online or over the phone.

2. Once your appointment is confirmed you will receive our patient intake forms to complete and send back within 24 hours before your visit. Please check your email.

3. On the day of your visit please bring or provide access to your unexpired ID card, and a list of all current medications (prescription and over-the-counter).

4. Expect your appointment to take between 20 minutes- 45 minutes, depending on the reason for your visit.

5. All CO-pays, CO-insurance, or full payment is required at or before the time of service.

6. If you need to cancel or reschedule your visit, please contact us at least 24 hours before your scheduled visit. You will be charged a no-show fee for late cancellation or failure to show up for an appointment. CALL (757)719-9039, or send us a message via the patient portal
